创伤（英语：Trauma），或称外伤、损伤（英语：Injury），是指外力对身体造成的伤害[1]。这可能是由意外事故、跌倒、撞击、武器等原因引起的[1]。重大创伤是可能导致长期失能或死亡的创伤。
2013年，全世界有480万人死于创伤，而1990年为430万人[2]。在这些死亡中，有30%以上是运输相关的伤害[2]。2013年，有36.7万名五岁以下儿童死于创伤，而1990年为77.6万[2]。创伤占全部死因中的9%，并且是世界上第六大死因[3][4]。
相关研究的学问称为创伤学。

世界卫生组织制定了《ICECI医疗分类码》（International Classification of External Causes of Injury），损伤可依照下列方式分类：
- 损伤的机制
- 造成损伤的物体或物质
- 损伤的位置
- 受伤时的活动
- 相关人的意图
以及其他模组。这些分类码可以识别各特定人群损伤的分布，以便更详细地研究病因和预防措施[5][6]。
美国劳动统计局开发了《OIICS职业伤害分类系统》（Occupational Injury and Illness Classification System），损伤可依照下列方式分类：

- 受伤的身体部位
- 来源
- 造成损伤的事件
OIICS于1992年首次发布，此后进行了数次更新[7]。
《OSICS运动伤害分类系统》（The Orchard Sports Injury Classification System）可分类损伤，用于研究特定的运动伤害[8]。
